1) Vermont State University - Physical Form (Once completed upload to your NEW STUDENT CHECKLIST, you will not need to submit the form to Athletics)

• Vermont State University Johnson Athletics and Your Insurance (Understand what is covered and what is not, VTSU School insurance option)
• All student-athletes must submit a front and back copy of their Health Insurance cards during Fall pre-season (Aug 15th) Check-in or prior to the start of practice for all other student-athletes 

2) Required NCAA Sickle Cell Testing Information and Disclosure Form (Where to find your test results or get tested)

3) Athletic Training Department under 18 year old Consent Form (if needed)
