Complete the Items Listed Below (1,2, 3 and 4 if needed) 

1) Vermont State University - Physical Form (Once completed upload to your NEW STUDENT CHECKLIST, you will not need to submit the form to Athletics)

• Vermont State University Johnson Athletics and Your Insurance (Understand what is covered and what is not, VTSU School insurance option)
• All student-athletes must submit a front and back copy of their Health Insurance cards during Fall pre-season (Aug 15th) Check-in or prior to the start of practice for all other student-athletes 

2) Required NCAA Sickle Cell Testing Information and Disclosure Form (Where to find your test results or get tested)

3) Concussion Testing: New Athletes 

• All New student-athletes must complete online concussion baseline testing prior to arriving for Fall pre-season (Aug 15th) or the start of practice for all other student-athletes

4) Athletic Training Department under 18 year old Consent Form (if needed)
